Middletown PBA Golf Outing 2014

     Middletown Twp PBA Local 124 is holding it's annual golf outing. Friday August 8th, at Pebble Creek Golf Club in Colts Neck. NJ. This year we have teamed up with Jason's Dreams for Kids. This event will help raise funds for both JDFK & Local 124.
    
Jason's Dreams for Kids, Inc. is a 501(c)(3) organization, which was founded in 1992 in memory of Jason Douglas Creager, who passed away on January 18, 1992 after losing his battle with cancer. The organization is devoted to granting wishes to children diagnosed with life-threatening illnesses. Bringing a little happiness and putting a few smiles on these children's faces is our goal - and hopefully, their parents faces too! You can check them out at www.jasonsdreamsforkids.
   Registration starts at 7:30 am and first tee time is 9AM. If you are intersted in participing please print out the attached flyer and mail in with payment to:

               $150.00 per person or $600.00 per foursome

If you are unable to attend the event and would still like to make a donation, please use the sponsor sheet and mail your donation to the address provided.
